We will make most of the preparations for your appointment, but there are a few things that you can do to help make things more efficient as well as more comfortable for you, the patient.

Making an Appointment

To make an appointment, you will need to give the scheduler the following information:

  • Patient name
  • Patient date of birth
  • Patient contact information
  • Reason for the appointment

To schedule an appointment, you can call one of the following numbers:

  • (541) 322-1765 for the Bariatric Program Coordinator
  • (541) 322-1756 for the Vein Program Coordinator

Cancelling an Appointment

We realize that sometimes life can get in the way, but we ask that if you do need to re-schedule or cancel an appointment that you give 24 hours notice.  Failure to do so could result in a service fee.

Preparation for your Appointment

When you come to your appointment, it is helpful to arrive early and if possible, wear loose, comfortable clothing that is non-restrictive in nature.  If you are coming in for a procedure such as a colonoscopy, it is particularly important to do these things.  You will be more comfortable, and the extra time prior to the procedure will allow you to thoroughly review the pre-procedure paperwork without feeling rushed.  The following is a checklist of what to bring with you:

  • Insurance card, including Medicare and Medicaid:  Having a current copy will help us to process your bill correctly and efficiently.
  • Photo Identification
  • Co-payment or pre-payment: If your insurance plan has a co-payment or you have a pre-payment requirement, this is required at the time of service.  Please check with the scheduler when making your appointment for more information.
  • List of medications:  It is helpful to our providers to know what medications that you are currently taking.  Please bring a list of all prescription drugs, over the counter medications, vitamins, or supplements that you may be taking, as well as a list of all known drug allergies.  Alternatively, you can fill out the medical history forms available here prior to your visit.
  • Medical records & pertinent test results (including x-rays, lab results & mammograms):  If you are being referred to us by another provider or if your doctor has ordered tests, please be sure to arrange to have these records sent to our offices.  Please call us at (541) 322-5753 for more information.
  • Primary Care Provider Information:  Please bring the name & phone number of your primary care provider.
  • Questions:  Feel free to bring a list of concerns or questions that you may have to discuss with your doctor.
  • Directions:  If you are new to the clinic and are unsure of where we are located, you can print out directions to either location to ensure that you will arrive on time.
